Behavioral health and substance use disorder

The PPO, EPO, and HDHP cover medically necessary in- and outpatient care for behavioral health and substance use disorder treatment.

You are encouraged to contact your service provider before beginning treatment with a therapist, although you're not required to do so. Your service provider can help match you with a provider who has the appropriate background and experience to address your concerns.

  • If you are in the PPO, your out-of-pocket costs will be lower if you choose network providers. If you choose a provider who is not part of the network and your service provider certifies that the treatment is medically necessary, eligible services will be covered at the out-of-network level.
  • If you are in the EPO or HDHP, you must use a network provider to access your behavioral health and substance use disorder benefits. If you choose an out-of-network provider, you will be responsible for 100 percent of the costs.

Find network providers

To find network providers, call the Member Service number on the back of your medical ID card or use the provider search tool on your service provider's website.

Pre-certification is required* for some behavioral health and substance use disorder services. Learn more about which services require pre-certification.
