One of the most important tools available on MyActiveHealth.com/pcusa to active members of the Medical Plan is your secure and confidential Personal Health Record (PHR).* Your PHR makes it easy for you to confidentially and conveniently manage your own health history, as well as your children's, by giving you a place to record key health information.

What Is a PHR?

A PHR is an online health record that conveniently does your health history record-keeping for you. It includes information you provide and automatically updates when your claims from Highmark, Express Scripts, and Quest Diagnostics (if you use its labs) are processed. This enables you to quickly access your current health history whenever you want.

When you launch your PHR, your recent health history will be organized in one convenient place, thereby helping you improve the effectiveness and safety of the care you and your family receive. It makes it easy to keep track of your:

  • doctor and hospital visits
  • immunizations
  • tests and procedures
  • medications
  • allergies
  • other health-related information

A PHR is available to each enrolled member in your family. (Dependent children age 17 and younger do not have direct access to their PHR, but parents/guardians can maintain their PHR.) Once you set up a PHR for you or your child, it automatically fills with health history details for the last three years, pulled from your Highmark and Express Scripts claims. You can then add other health information yourself.

You can share your PHR with your spouse or doctor, but it's at your discretion - no one can gain access without your authorization. You can also use your PHR to create reminders about exams and screenings, print out a health summary for a new doctor, and much more.

Launch your PHR simply by creating your account on MyActiveHealth.com/pcusa. In addition to the information from Medical Plan claims that will automatically populate, you can take the online Health Assessment at MyActiveHealth.com/pcusa to add details only you would know.

The Health Assessment

You can establish the foundation of your PHR by completing the Health Assessment on MyActiveHealth.com/pcusa. This tool gives you an objective evaluation of your health strengths and any self-reported issues or potential causes for concern. The information is then used to help populate your PHR.

So put aside 15 minutes to take this easy-to-use Health Assessment and learn about the changes you can make to improve your health.

How to Sign In

You have two options for signing in to MyActiveHealth.com/pcusa.

  1. Log in to Benefits Connect and select “Medical” under “My Benefits.” Then click “View Personal Health Record” or “View Health Management Portal” under “Partner Resources.” Accessing the site through Benefits Connect simplifies the sign in process – you do not need to register for MyActiveHealth.com/pcusa.
  2. Visit MyActiveHealth.com/pcusa. If you go directly to this site, you will be required to register.

Private and Confidential

ActiveHealth Management and the Board of Pensions comply with all federal and state privacy laws to keep your personal health information confidential. You control who views your PHR; the Board of Pensions and your employing organization cannot access it.

*Not available to members enrolled in Anthem, CIGNA International Expatriate Benefits, or Triple-S, or to retired members covered by the Medicare Supplement Plan.
