Eligibility and Coverage Options

The Optional Dental Program, a group dental program offered through Aetna, is available to Plan members with medical coverage and to Affiliated Benefits Program members whose employers offer the program to them. Depending on where Plan members live in relationship to the Aetna provider network availability, they are eligible to enroll for one of the following:

• Dual Option-DMO (dental maintenance organization) or PPO (preferred provider organization)
• PPO only
• Passive PPO (indemnity) only

Enrollment

Members can enroll:

• when they first enroll in the Benefits Plan
• during the Board of Pensions' open enrollment periods
• within 31 days of certain life-change events

Dental enrollment forms are available by calling the Board at 800-773-7752 (800-PRESPLAN). The Board will review member records to determine eligibility and the coverage options available.

Orthodontics

Orthodontic coverage for children prior to the age of 19 is available after three consecutive years of participation in the Dental Program.
