The Medical Continuation Program gives you the option of continuing essentially the same healthcare coverage you and your family had before the termination of your Plan membership, but on a self-paid basis and for a limited time period. Your deductible and copayment amounts are based on the church-wide median salary. Please see the Healthcare Coverage booklet for details.
Eligibility
Those eligible to subscribe to the program include:
- Members and seminary students terminating from the Benefits Plan
- Members on the Transitional Participation Basis
- Members for whom disability status ends and terminated status begins
- Retired members under 65
- Surviving and divorced spouses
- Spouses and eligible dependent children of formerly active members (even if the member does not subscribe)
- Spouses and eligible dependent children of retirees(even if the member does not subscribe)
- Dependent children who lose eligibility while covered under parent

To Subscribe
To activate coverage, please complete the Medical Continuation Subscription/Waiver form and send it to the Board within 60 days of the event that results in your termination of coverage. The initial payment is then due within 45 days of submitting the form. Coverage will not be effective until the initial payment is received. Any free coverage period begins immediately following termination of service.
