Case Management

You can get help managing your serious health condition. If you are beginning or recovering from a hospital admission, struggling with multiple medical problems, or facing catastrophic care needs, you can receive help from a Highmark nurse case manager, at no cost to you.

Your case manager can

  • provide educational support;
  • assist in coordinating needed healthcare services;
  • put you in touch with community resources;
  • assist in addressing obstacles to your recovery, such as benefit and caregiver issues; and
  • answer your questions.

A nurse case manager may contact you, or you may contact Highmark using the Member Service number on the back of your ID card (888-835-2959).
