You can get help managing your serious health condition. If you are beginning or recovering from a hospital admission, struggling with multiple medical problems, or facing catastrophic care needs, you can receive help from a Highmark nurse case manager, at no cost to you.
Your case manager can
- provide educational support;
- assist in coordinating needed healthcare services;
- put you in touch with community resources;
- assist in addressing obstacles to your recovery, such as benefit and caregiver issues; and
- answer your questions.
