We encourage you to notify the Board as soon as possible in the event of a member’s death or the death of a member’s spouse or child.
Death of a Member
Call the Board at 800-773-7752 (800-PRESPLAN) and provide the representative with the following information:
- name of member
- date of death
If you are an eligible spouse, eligible child, or designated beneficiary, you will receive a letter from the Board asking that you complete, sign, and submit a Death Benefits Claim form and a death certificate to the Board to ensure timely payment of benefits.
Medical Coverage for Eligible Family Members
If you are the surviving spouse or eligible dependent of a member who was active or enrolled in transitional participation coverage at the time of death, and was covered under the Pension, Death and Disability, and Medical plans, and you were covered under the Medical Plan, your medical coverage will continue for one year from the date of death as long as you are eligible.
Neither you nor the employer will be billed for this coverage. After the year, you may enroll for medical continuation coverage for an additional 24 months or until you are eligible for Medicare, whichever is sooner or, if a dependent child, until you reach age 26.
If you are the surviving spouse or eligible dependent of a member who was active or enrolled in transitional participation coverage at the time of death, and was not covered under the Pension and Death and Disability plans, and you were covered under the Medical Plan, you may enroll for medical continuation coverage for 36 months from the date of death or until you are eligible for Medicare, whichever is sooner or, if a dependent child, until you reach age 26.
If you are the surviving spouse or eligible dependent of a disabled member and you were covered under the Medical Plan at the time of his or her death, your eligibility for medical coverage totals three years from the date disability began or until what would have been the member’s normal retirement date, whichever is sooner.
For example, if the member had been disabled for one year at the time of death, you would be eligible for medical coverage for two years after his or her death, unless the retirement date occurred before that. You would be responsible for paying any monthly contribution the member had been making for eligible family coverage.

If you are a surviving spouse of a retired member, and you were enrolled in either medical continuation coverage or the Medicare Supplement Plan at the time of the member’s death, you are eligible to continue coverage for your lifetime, through medical continuation and then the Medicare Supplement Plan when you turn 65.
If you are not yet eligible for Medicare, you can enroll for medical continuation coverage until you become eligible for Medicare. You can then enroll in the Medicare Supplement Plan if you are participating in Medicare Part A and Part B.
If you are under age 25 and the surviving dependent child of an active member who was enrolled in the Death and Disability Plan at the time of death or a retiree who initiated Pension Plan payments immediately upon termination of pension coverage, you may be eligible for a post-high school education grant. You will be sent information regarding this benefit when the member’s death is reported.
Death of a Spouse or Child
If a member’s spouse or child dies, contact the Board at 800-773-7752 (800-PRESPLAN) with the following information:
- name of the spouse or child
- date of death
If the family member had medical coverage, the coverage will end. If the member has medical coverage through menu options, if applicable, monthly contributions will be adjusted accordingly.
If the spouse or child was enrolled for Supplemental Death Benefits, you must submit a death certificate for payment of the benefit. You may also want to consider updating your beneficiary designations, available through Benefits Connect.
