We encourage you to notify the Board and the presbytery as soon as possible in the event of a member’s death or the death of a member’s spouse or child.
Death of a Member
Please call the Board at 800-773-7752 (800-PRESPLAN) and provide a service representative with the following information:
- name of member
- date of death
If you are an eligible spouse, child, or designated beneficiary, you will receive a letter from the Board asking that you complete, sign, and submit a Death Benefits Claim form and a death certificate to the Board to ensure timely payment of benefits.
Medical Coverage for Eligible Family Members
If you are the surviving spouse or eligible dependent of a Traditional Program member who was active or disabled at the time of death and covered under the Medical Plan, you will receive one year of free medical coverage. After this time, you will have the opportunity to enroll for medical benefits for as long as needed. At age 65 and older, you may subscribe for coverage in the Medicare Supplement Plan through the Board if you meet the eligibility requirements.
If you are a surviving spouse of a retired member and you were enrolled in either the Medical Continuation Program or Medicare Supplement Plan at the time of the member's death, you are eligible to continue to enroll for medical coverage for your lifetime.
If you are not yet eligible for Medicare, you can enroll for coverage under the Medical Continuation Program until you become eligible for Medicare. You can then enroll in the Medicare Supplement Plan if you have maintained continuous coverage under the Medical Plan (or a waiver has been executed and approved by the Board) and are participating in Medicare Parts A and B.
If you are a surviving dependent child covered under one of these plans, you can continue coverage upon the member’s death until age 26 by paying the monthly cost of coverage. You may also be eligible for an education grant to assist paying for post-secondary education; you will be sent information regarding this benefit when the death of the member is reported.
Death of a Spouse or Child
Please call the Board (or have someone contact us on your behalf) at 800-773-7752 (800-PRESPLAN) and provide a service representative with the following information:
- name of the spouse or child
- date of death
If your spouse or child
- had health coverage through your benefits, his or her coverage will stop and your monthly contribution will be adjusted, if applicable, automatically.
- was participating in the Supplemental Death Benefit Plan, a death certificate is required before the benefit is payable.
- was named as a beneficiary for any of your benefits, you can complete and return the appropriate forms to name new beneficiaries.
The Board offers support to help members cope with grief and loss.
